INITIATING STRUCTURE: Veterinary Pharmacy Service.
CONDITION TO BE FULFILLED:
- be a Veterinarian registered with the National Order of Veterinarians or a Pharmacist registered with the National Order of Pharmacists.
- have authorization to open an establishment intended for this activity
COMPOSITION OF THE FILE:
Document to consult: table of the National Order of Veterinarians or the table of the National Order of Pharmacists.
Documents to provide:
- Application stamped at the current rate specifying the name of the trial sponsor, the quantity to be imported, the destination of the product and the reference to the experiment or the authorization for experimentation;
- Administrative authorization to open the establishment intended for this activity;
- Certificate of effective operation dated less than three months;
- Certificate of non-practice of veterinary medicine in private practice for veterinarians and
of non-keeping of a pharmacy for pharmacists respectively issued by the
national councils of the orders on which they depend; - Certificate of registration with the professional body on which the applicant belongs;
- Authorization to practice in private clientele;
- Certificate of compliance with veterinary medical ethics;
- Import license issued by the ministry responsible for trade.
SIGNATORY OF THE ACT: The Minister.
TIME LIMIT: Thirty (30) days.
PROVISION TERMS: Notification/withdrawal.
