INITIATING STRUCTURE: Veterinary Pharmacy Service
CONDITIONS TO BE FULFILLED:
- Be a Veterinarian registered with the National Order of Veterinarians or a Pharmacist registered with the National Order of Pharmacists;
- Have authorization to open an establishment intended for this activity
COMPOSITION OF THE FILE:
Document to consult: Table of the National Order of Veterinarians or the table of the National Order of Pharmacists.
Documents to provide:
- Request stamped at the current rate;
- Administrative authorization to open the establishment intended for this activity;
- Certificate of effective operation dated less than three months;
- Certificate of non-practice of veterinary medicine in private practice for veterinarians and non-practice
- Maintaining a pharmacy for pharmacists respectively issued by the national councils of the orders on which they depend;
- Certificate of registration with the professional body on which the applicant belongs;
- Authorization to practice in private clientele;
- Import license issued by the ministry responsible for trade.
SIGNATORY OF THE ACT: The Minister.
TIME LIMIT: Sixty (60) days.
PROVISION TERMS: Notification/withdrawal.
